At your first visit to PACI we will discuss important background information and allow time for you to get to know your doctor. We strive to provide clear explanations of your diagnosis and treatment options so you can make educated decisions regarding your health care.

At PACI we want to make your visit as productive and efficient as possible. To help us make the most of your time, please bring the following to your first appointment:

• Insurance card and picture identification
• Name of your primary care physician, date last seen by primary care physician and any previous podiatrists
• Medical records or x-rays relating to your current or previous foot problems, if you have them
• Completed New Patient Information Form Click Here

Please arrive at least 15 minutes prior to your scheduled appointment time to allow us to complete your registration process. We look forward to seeing you soon!

If an emergency should occur after office hours, please call (513) 474-4450 and leave a message with your name, call back number and emergency.  The physician on call will be paged with your message and will call you back as soon as possible.  One of our doctors is always on call and available 24/7.

We accept most major insurance companies and Visa, MasterCard, and Discover credit cards.
If you have any billing or insurance questions, please call our Billing Office at 513-474-1847.   That is what they are there for and are happy to help you in any way that they can.

Appointment Policy

Due to our Doctors’ increasingly busy schedules, please note that if you are more than 15 minutes late for your appointment, you will be asked to reschedule.  We appreciate your understanding, and ask that you plan accordingly when scheduling appointments to ensure you can arrive on time.
